WELCOME and Thanks for visiting the South Cameron High School ELIGIBILITY CENTER.
The Louisiana High School Athletics Association (LHSAA) and South Cameron High School (SCHS) require certain forms to be on file in the Athletic Director's office PRIOR to participation on any athletic team(s).
The forms below, along with copies of the birth certificate and transcript, are MANDATORY for every student athlete to have COMPLETED and on file (hard copies and .pdf copies) with the Athletic Director PRIOR to competition in any sport. Feel free to download these forms as needed.
STEP 1: Click on the forms below and print them
STEP 2: Get a copy of the student athlete's BIRTH CERTIFICATE (this is the Coach's responsibility)
STEP 3: Get a copy of the student athlete's TRANSCRIPT from Counselor or Athletic Director
STEP 4: Put in a folder and keep the hard copies
STEP 5: Scan documents and send to Athletic Director
